Original Entry Form

This multi-purpose form can be used as a simple receipt for payments received or as a work order for the superintendent, the funeral director or a grave digger when a commercial contractor is used. The triplicate form allows one copy to be retained in the cemetery office or rectory (white copy), one copy for the family (buff copy) and a third copy (pink copy) for use as a work order.
