Please Note: It is important to the success of the program that you provide the information requested on the survey sheet. Make copies of the survey and distribute to your Parish Cemeteries PLEASE! Please return the survey sheets no later than March 15, 2013. The information received will be used to develop press releases which will be made available to your local media outlets to publicize the events at your cemetery.
